Credit application for Arizona Customers
The Arizona Department of Revenue requires that this certificate be filled out by purchaser for tax-exempt transactions. The certificate must be furnished to the vendor.
California State Board of Equalization requires that sales tax be charged on all products sold in the state. Customer's shipped orders require either sales tax or this signed resale card showing a California resale number.
